Time for Back-to-School Health Forms and Physicals

Back to SchoolBeat the rush and make an appointment now for back-to-school physicals and immunizations. Your pediatrician must see your child before we can complete and sign the state-required health forms for school and after-school programs and day care. We cannot accept faxed or phone requests to complete health forms.

Staten Island public schools resume Sept. 9., and your child must have current health forms and current immunizations to attend. New York also requires all new students and children in kindergarten, second and fourth grade to have current physical exams. Bring any school or sports forms you have to your child’s appointment, and we will complete them and give them back to you the same day. We also have state-required forms in our office. If your child takes medication during the school day, you will need a medication administration form on file with the school nurse. If your child has had a physical within the last 30 days, you may call and speak to a nurse about completing school forms.
