Summer camps will be starting soon and your child will need current medical forms and up-to-date shot records to attend. Don’t wait until the last minute. If your child has not had a physical within 30 days, he or she will need to see a CHCR medical provider for an exam before we can complete your camp medical forms. State law requires that all children have current TB skin tests (PPD) and up-to-date vaccinations. Call now to make an appointment for a physical.
If your child has had a physical within 30 days and his or her shots and TB skin test are up to date, you may drop off a form for our staff to complete. Dropped-off forms will be mailed to you in 7 to 10 days. You will receive a completed form at your visit if your child has a physical. Also, now is the time to think about medical forms needed for school. Save yourself the trouble and an extra trip in September and bring your school forms in early when you bring your child for a camp physical.
